Any Fort Plain Central School student in grades 7-12 who is interested in having their sports physical completed at school for 2021-22 school year should report to the high school nurse’s office on Tuesday, Aug. 3, from 8-10 a.m. for the pre-physical screening. The student will need to return to the nurse’s office on Wednesday, Aug. 4, at 8:30 a.m. to see the doctor.
The district has received an inquiry about whether physical exams can be completed by a student’s personal doctor. Athletic Director Charlie Karker: “The answer is yes, however they also need the following:
1) An updated health history form
2) Urinalysis, height, weight, and vital signs on the form from their doctor
3)AND their own doctor’s physical exam will still need approval from the school doctor per NYS regulations, prior to practicing.
If they choose to have their physician do the physical, it would be best if they turn in their completed paperwork on Aug. 3 or Aug. 4 to Tara Hayes, Jr./Sr. High School nurse.”
